With a person-centered planning & thinking approach, it is our vision to be recognized as an organization that transforms individuals’ lives and provides families the peace of mind in knowing their loved one has a voice and a valued role in their community.Role Summary:Conducts medical file reviews of referrals to the local regional center intake and assessment unit for eligibility under the Lanterman Act. The medical file review includes examining pertinent medical records and may involve consultation with the assessment team and health care providers. Key Responsibilities:
- Works with community professionals and agencies towards increasing the developmental disability care expertise in the community.
- Participates in efforts to upgrade and enhance programs for clients in general.
- Participates with and provides consultation to case managers and interdisciplinary teams, to make recommendations for on-going case management.
- Ability to exercise independent and sound judgement, utilizing all relevant information in decision making.
- One year’s experience with persons with developmental disabilities.
- Demonstrate cultural awareness and sensitivity and an ability to work effectively with culturally diverse populations.
- Must be computer knowledgeable
License:
- Currently holds a Board-certified Nurse Practitioner license
