As One Medical for Seniors High Risk Programs continues to expand, we are seeking a High Risk Tele-Assist Scheduler to join the newly formed Tele-Assist Program team. Reporting to the Tele-Assist Program Operations Manager, this person will support both patients and in office care teams by coordinating the schedule of Tele-Assist visits across multiple markets in partnership with other High Risk Care programs, managing in-bound and completing outbound calls to support care coordination, tracking and monitoring One Medical Senior Health patients discharged from Emergency Room (ER) visits/stays, acute, and post-acute stays, and communicating with the patient to ensure the appropriate follow-ups and prevention of readmission.

You have mastered the art of customer-service, complex scheduling tasks, administrative work, and are passionate about Seniors’ health. You are comfortable with the ambiguity that comes with joining a new team, and are excited about the opportunity to collaborate on developing and optimizing workflows.
What you'll likely work on:
- Facilitate the scheduling of multiple Field Care Coordinators and virtual Nurse Practitioners across multiple markets (AZ, WA, TX, GA & CO) as they provide a variety of patient care visits via Tele-Assist care model
- Field incoming phone calls/messages from patients and their families and appropriately address those needs or provide hand off to other key team members in an appropriately urgent manner
- Assist the care coordination team, primary care offices, and patients with care coordination for ER visits/stays, acute and post-acute care facility admissions and discharges.
- Act as a liaison with care teams and outside medical providers where appropriate.
- Work collaboratively with hospitals, post-acute facilities, and home health organizations
- Monitor acute and post-acute care facility admissions, lengths of stay and clinical outcome and provide feedback as needed.
- Coordinates discharge, follow up appointments, and nurse follow up calls as appropriate
- Engage in process building and development for a new team approach by listening, sharing input and ideas, and enacting decisions and plans.

What you’ll need:
- At least one year in a patient facing healthcare role
- At least 1 year of experience in scheduling or dispatch process management in healthcare or other settings (retail, logistics, service industry, etc.)
- A proven track record of persisting through change, demonstrating a forward thinking perspective when under pressure, and consistently stepping up to take action on challenges
- Strong written and verbal communication skills; Spanish Speaking preferred
- Experience as a hospital case management assistant, home health care coordinator, or transition navigator preferred
- Understanding of Mac iOS, Google suite.
- Experience working with One Medical Seniors preferred.
This is a full-time hourly position (40 hours/week) working remotely with patients and teams. This person must be willing to work East Coast hours, Monday - Friday, 8:00am - 5:00pm.
One Medical is committed to fair and equitable compensation practice. The base compensation range for this role is $22.50 to $28.50 per hour. However, actual compensation packages are based on several factors that are unique to each candidate. These factors include, but are not limited to, job related knowledge and skill set, depth of experience, certifications and/or degrees, and specific work location.
The total compensation package for certain roles may also include additional components such as a sign-on bonus, annual performance bonus, equity grants in the form of RSUs, medical and other benefits and/or other applicable incentive compensation plans.
