Job Description:
The Clinical RN Liaison plays a key role in bridging hospital care with home-based therapies. This position supports discharge planning, patient education, and care transition for individuals requiring Home IV therapy or enteral nutrition. The Liaison also serves as a clinical representative for CarepathRx within assigned hospitals, coordinating with hospital teams, promoting CarepathRx services, and ensuring safe, high-quality transitions to home care.
Essential Functions

Clinical Responsibilities
-
Perform infusion and skilled nursing care to patients prior to discharge, as needed.

Conduct pre-discharge assessments and patient education on home therapy plans.

Communicate with physicians and interdisciplinary teams regarding patient care.

Provide leadership in nursing practice, prioritizing patient safety and clinical excellence.

Complete all required clinical documentation in a timely and accurate manner.

Take appropriate safety precautions when handling hazardous materials (e.g., cytotoxic agents, blood and body fluids).

Act as a central liaison between hospital teams and CarepathRx operations to facilitate efficient, safe discharges.

Coordinate with home health agencies, payers, and the CarepathRx team to schedule services and ensure readiness for discharge.

Discuss financial responsibilities with patients and assist in setting expectations around home therapy.

Represent CarepathRx in hospital committees and meetings related to patient education, referral processes, or discharge strategy.

Collaborate with clinical leaders, case managers, and provider offices to promote CarepathRx capabilities and drive referrals.

Support internal sales and marketing teams by delivering education and facilitating referrals from clinical staff.

Maintain RN licensure, CPR certification, and all required competencies.

Adhere to HIPAA standards and maintain patient confidentiality at all times.

Attend required training, in-services, and team meetings.

Demonstrate a professional appearance, courteous demeanor, and effective communication in all interactions.

Take initiative to present ideas and support continuous improvement of clinical operations.

Requirements
