Job Description Summary:
Responsible for the compounding and dispensing of prescribed medication, including I.V. admixture, providing professional judgment, monitoring and implementing the unit dose system of medication distribution. This person is competent to handle Neonate, Pediatric, Adolescent, Adult and Geriatric patients. Pharmacy degree from an institution accredited by the American Council on Pharmacy Education required. Current licensure as a Pharmacist by the State of Tennessee. Experience in a hospital setting preferred. Under the general supervision of the Pharmacy Manager, responsible for assigned shift as scheduled full time or part time. Employee is subject to call back and overtime as required by the hospital.
ESSENTIAL JOB FUNCTIONS:
- Analyzes and enter into the computer physician medication orders, determining any contraindications or allergies and resolution of any problems.
- Prepares all IV admixtures, including hyperalimentation and preparation of antineoplastics, determining and resolving any incompatibilities or contraindications.
- Generates or maintains proper documentation of records of drugs dispensed in accordance with state federal law and hospital policies and procedures.
- Provides information including calculations, drug information, and proper protocol to physicians and nurses whenever requested.
- Manufactures and dispenses prescribed medication for patient and employee prescriptions, in compliance with hospital policy.
- Audits medications in stock to ensure the proper storage, the isolation and proper disposal of out of date medication, and separation of external and internal medication.
- Checks all unit dose cars and packaging, and corrects errors or deficiencies.
- Assigns and monitors work of Pharmacy Technicians.
- Contributes to a positive work climate and the overall team effort of the department.
- Performs related responsibilities as required or directed.
JOB SPECIFICATIONS:
EDUCATION:
- Pharmacy degree from an institution accredited by the Accreditation Council for Pharmacy Education required or provide proof of graduation before beginning employment as a Pharmacist.
- To practice under a collaborative practice pharmacy agreement, the pharmacist must have completed a PGY-1 pharmacy residency or achieved the Board of Pharmacy Specialties certification. PGY-2 pharmacy residency is preferred. Pharmacists must complete a minimum of 10 hours of ACPE accredited continuing education in their clinical practice area.
LICENSURE, REGISTRATION, CERTIFICATION:
- Current licensure as a Pharmacist by the State of Tennessee.
EXPERIENCE:
- Experience in a hospital setting preferred.

- Skill and proficiency in the principles, concepts and techniques of the pharmacy profession.
