Assessor Reviewer RN - Grand Junction, CO
Job Overview
This position is responsible for conducting face-to-face assessments of members who may need skilled nurse assessment services. The incumbent completes all assessments using a skilled care acuity tool and provides a recommendation for services based on members health care needs. Members who are assessed reside in the community or are ready to be discharged from a facility.
The incumbent may also serve as mentor or preceptor for new staff, assisting in the formal training of assessors, complete quality assurance audits, inter-rater-reliability reviews, and may be involved in process improvement initiatives.
Essential Functions
- Performs face-to-face and/or virtual assessments of persons with disabilities. Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
- Documents assessment results for the individual member status or negotiates service plan as needed.
- Completes quality assurance competency reviews and other tasks/projects as assigned such as new assessor training.
- Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care.
- Performs miscellaneous duties as assigned.
Requirements
- Four-year nursing degree and at least three years of experience in nursing.
- Experience working with individuals with disabilities, individuals with intellectual and developmental disabilities (IDD).
- Experience working with Pediatric and Geriatric populations with various disabilities, and individuals with complex health needs preferred.
- Current and unrestricted RN licensure in Colorado or a compact nursing license preferred.
- Bilingual or Spanish speaking preferred.
- This individual will be expected to have occasional travel to surrounding areas.
