St. Joseph’s Villa is seeking a compassionate and resourceful Youth Rapid Rehousing Case Manager to join our Flagler Housing & Homeless Services team. This position supports young adults ages 18–24 who are experiencing homelessness by helping them secure permanent housing and develop the skills, resources, and stability needed to remain successfully housed.

The Case Manager works as part of a collaborative Rapid Rehousing team and partners with clients, landlords, community organizations, and service providers to address barriers to housing and support long-term stability.

This role is ideal for someone who is passionate about housing stability, youth services, case management, and helping individuals achieve independence.

Responsibilities

Housing & Client Support

• Assist young adults with obtaining and maintaining safe, affordable rental housing in the community.
• Participate in client intakes and assessments to identify housing barriers and develop individualized housing stabilization plans.
• Assist with temporary housing arrangements when needed.
• Help clients coordinate move-in activities, including utility deposits, furniture, moving assistance, and household supplies.
• Build relationships with landlords and community partners to expand housing opportunities for young adults with limited or negative rental histories.

Case Management & Housing Stabilization

• Provide individualized case management and connect clients with appropriate community resources.
• Help clients establish realistic goals related to housing, employment, income, education, budgeting, and independent living.
• Develop housing stabilization plans and support clients in taking concrete steps toward long-term stability.
• Educate clients about tenant rights and responsibilities, program expectations, and available benefits.
• Provide referrals to health, medical, employment, education, financial, and other community resources.
• Follow up with clients and community partners to ensure referrals and services are successfully connected.

Financial Assistance & Documentation

• Process financial assistance requests related to rental subsidies, utility deposits, utilities, furniture, and other eligible housing expenses.
• Prepare check requests, invoices, and supporting documentation for client financial assistance.
• Maintain accurate and timely client records in accordance with program, funding, and regulatory requirements.
• Maintain required paper files and electronic records, including HMIS/HCIS documentation.
• Track client progress, services provided, and program expenditures.
• Complete required assessments and documentation used to measure program outcomes.

Team & Community Collaboration

• Participate in case coordination meetings, team meetings, community meetings, and group activities.
• Collaborate with community agencies and coordinated entry partners to support client success.
• Participate in CACH Coordinated Entry conference calls and care coordination meetings.
• Present client progress, barriers, and recommended solutions to the team.
• Develop and maintain relationships with landlords, service providers, and community organizations.
• Identify and cultivate new community resources that can benefit program participants.
• Participate in required training and professional development activities.

Qualifications

• At least one year of directly related experience working with a similar population.
• Bachelor’s degree in Human Services or a related field preferred; relevant experience may be considered in lieu of a degree.
• Valid Virginia driver's license with an acceptable driving record.
• Proficiency with Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
• Ability to successfully complete required background checks and agency-required training.
• Ability to travel locally for client visits, meetings, trainings, and community activities.
