Registered Nurse (RN) – Home Health Care - Full Time
Pay Range: $43.27 – $58.68 per hour (based on years of experience)

Job Summary:
Provide skilled nursing care in patients’ homes following physician orders and a comprehensive plan of care. Serve as the clinical lead during home visits, perform assessments, and coordinate care across the care team.

Qualifications:

• Current and unrestricted New Jersey RN license.

• One to two years of recent acute care or community health experience.

• Baccalaureate degree in nursing preferred.

Registered Nurse (RN) – Hospice – Full Time
Pay Range: $43.31 - $60.16 per hour (based on years of experience)

Job Summary:
Provide compassionate, patient-centered hospice nursing care in patients’ homes to ensure comfort and symptom management at end of life. Serve as a key member of the interdisciplinary care team by assessing patient needs, implementing individualized plans of care, coordinating services, and supporting patients and families emotionally, spiritually, and clinically. Travel statewide to deliver in-home hospice services.

Qualifications:

• Current and unrestricted New Jersey RN license.

• Minimum of 2–3 years of hospice nursing experience; home care experience preferred.

• Graduate of an accredited nursing program; BSN preferred.

• Current CPR and BLS certification.

• Valid driver’s license and proof of auto insurance.

• Strong clinical judgment and assessment skills.

• Knowledge of the physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.

Registered Nurse (RN) – Premium Weekend, Home Health – Part Time

Pay Range: $43.27 – $58.68 per hour (based on years of experience)

*Participants will receive a 30% premium on top of their hourly rate.

Job Summary:
Provide skilled, compassionate nursing care to patients in their homes while promoting independence, wellness, and quality outcomes. Responsible for assessing, planning, implementing, and evaluating individualized patient care plans in collaboration with physicians, caregivers, and interdisciplinary team members. Educate patients and families on disease management, medications, safety, and ongoing care needs while ensuring efficient coordination of services and supervision of Home Health Aides.

Qualifications:

• Current and unrestricted New Jersey RN license.

• Minimum of 1–2 years of recent acute care hospital experience (Med/Surg, ER, ICU, OB/GYN, Peds, or specialty unit) and/or recent community health or home care experience.

• Graduate of an accredited nursing program; BSN preferred.

• Strong clinical assessment, communication, and care coordination skills.

• Ability to work independently in a home health setting while collaborating with an interdisciplinary team.

• Valid driver’s license and reliable transportation required.

Clinical Care Manager – Home Health – Full Time

Annual Salary: $89,574 - $139,253 (based on experience)

Job Summary:

Virtua Health is seeking an experienced Clinical Care Manager to support high-quality home health patient care through clinical oversight, documentation review, staff education, and interdisciplinary collaboration. This role serves as a resource to clinicians by promoting best practices in care planning, regulatory compliance, quality improvement, and efficient care coordination.

Responsibilities include reviewing and improving clinical documentation and care plans, educating clinicians during orientation and onboarding, conducting recertification reviews, and partnering with operational and IT teams to optimize workflows and Epic care plan functionality.

Qualifications:

• Current and unrestricted New Jersey RN license.

• BSN required; graduate of an accredited School of Nursing.

• Minimum of 3–5 years of home care experience required.

• Minimum of 2 years of experience in a specialty clinical area.

• OASIS coding experience required.

• Current BLS certification required.

• Strong knowledge of clinical documentation standards, care planning, utilization management, and regulatory compliance.

• Excellent verbal, written, organizational, and interpersonal communication skills.

• Ability to lead education initiatives, collaborate across teams, and support process improvement efforts.

Registered Nurse (RN), Oasis Reviewer– Home Health – Part Time (35 hours)

Pay Range: $44.57 - $60.44
Job Summary:

Provide skilled, compassionate nursing care to patients in their homes through assessment, care planning, treatment implementation, and ongoing evaluation of patient outcomes. Collaborate closely with physicians, interdisciplinary team members, patients, and caregivers to coordinate individualized plans of care while promoting quality outcomes and efficient utilization of agency resources.

Responsibilities include conducting comprehensive patient assessments, administering and documenting treatments, educating patients and families, supervising Home Health Aides, and ensuring compliance with documentation and patient safety standards.

Qualifications:

• Current and unrestricted New Jersey RN license.

• Minimum of 1–2 years of recent acute care hospital experience and/or recent home health or community health experience required.

• Home Health Care experience required.

• Coding and OASIS experience preferred.

• Strong clinical assessment, documentation, and communication skills.

• Graduate of an accredited nursing program; BSN preferred.

Home Health Aide (HHA) – Full Time – $2,000 Sign-On Bonus
Pay Range: $17.65 – $22.87 per hour (based on experience)

Job Summary:
Provide compassionate in-home personal care and support services under the supervision of a registered nurse to help patients maintain health, comfort, and daily functioning. Duties include assisting with bathing, dressing, feeding, mobility, light housekeeping, and documentation of care delivered. Travel within Burlington County is required as part of patient visit assignments.

Qualifications:

• Current New Jersey Home Health Aide (HHA) certification required.

• Valid, unrestricted driver’s license, reliable transportation, and auto insurance required.

• Current CPR and BLS certification required.

• High school diploma or equivalent preferred.

• Home Health Aide or Nurse Aide experience preferred.

Hourly Rate: $18.54 - $139,253.36
The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
