TSS & Intake Specialist

**This position is located in Melville, NY. It designated as an on-site role and requires regular in-office attendance during scheduled work hours.**

Position Summary

The TSS & Intake Specialist supports the successful delivery of the Heali Program and Tenancy Supportive Services (TSS) Program. This position manages intake, screening, and service navigation for Social Care Network members while maintaining an assigned caseload of TSS clients. The Specialist is responsible for verifying eligibility, maintaining accurate case documentation, tracking client progress, and ensuring services are invoiced promptly and accurately. 

Responsibilities:

• Manage the intake process for individuals seeking services through the Social Care Network.
• Conduct member screenings, assess identified needs, and connect members with appropriate programs, services, and community resources. Clearly explain available services, eligibility requirements, required documentation, and next steps to members. 
• Maintain complete, accurate, and timely electronic case files, case notes, service records, and supporting documentation.
• Submit accurate and timely invoices for eligible services in accordance with program requirements. 
• Maintain documentation of all programmatic activities, milestones and results.
• Manage a case load of TSS clients.
• Collect, review, and verify all documentation required to confirm client eligibility for TSS program.
• Maintain and regularly update TSS Tracker with client progress.
• Create, maintain, and update reports and trackers for all Heali based programs.

 Requirements: 

• Bachelor's degree
• Proficient in Microsoft Suite (Outlook, Teams, Excel, Word, PowerPoint) 
• Exceptional interpersonal skills, capable of working well with others 
• Proactive, detail-oriented with strong organizational skills 
• Strong written and oral communication skills 
• Strong research, analytical, and problem-solving skills
• Motivated self-starter able to work independently and remotely as needed
• Flexibility and desire to work on varying planning projects
