Responsibilities
- Conduct comprehensive speech-language assessments for residents in a long-term care setting.
- Develop, implement, and adjust individualized treatment plans to improve communication and swallowing function.
- Provide evidence-based therapy to residents, including both individual and group sessions as appropriate.
- Document evaluation findings, treatment plans, progress notes, and discharge summaries in the facility’s EMR system (Epic/EMR).
- Collaborate daily with an interdisciplinary team to integrate therapy goals with overall resident care and PDPM goals.
- Educate residents, families, and staff on communication strategies, dysphagia management, and progress toward goals; ensure clear, compassionate communication.

Requirements
- Current New York State license as a Speech-Language Pathologist.
- Eligible for or holds the Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
- Degree in Speech-Language Pathology from an ASHA-accredited program.
- Minimum LTC experience required.
- Proficiency with MS Office and Epic/EMR software.
- Knowledge of PDPM reimbursement system.
- Ability to work effectively in a long-term care setting.
- Travel eligibility: within a 75-mile radius; local candidates may qualify for the local rate.
