Responsibilities
- Develop, implement and evaluate health plan care management service lines
- Promote continuity of care across in-patient and out-patient settings to support whole-person care
- Coordinate care integration, case management, utilization review and care planning
- Collaborate with multidisciplinary teams to optimize member health outcomes
- Monitor member progress and adjust care plans accordingly
- Ensure adherence to program policies, regulatory requirements and quality standards

Requirements
- Current California RN license
- Experience in health plan care management, care coordination, utilization review and care planning
- Strong clinical assessment, documentation and communication skills
- Ability to work a day shift (8 hours) and participate in a travel assignment
- RN degree (ADN or BSN)
