Travel Registered Nurse Case Management Job

Overview
Registered Nurse – Case Management travel assignment in Saint Helena, California. Start date 10/12/2026 for 13 weeks. Weekly pay range of $2,262 to $2,358 per week, guaranteed 40 hours per week. Responsibilities
- Plan, organize, and direct patient care in the hospital setting to meet identified goals and the patient’s plan of care; work with the interdisciplinary team to coordinate services.
- Perform ongoing nursing assessments and document patient status, responses to treatment, and changes in condition; update the plan of care accordingly.
- Communicate clearly with patients, families, and care team members about care goals, education needs, rights and responsibilities, and discharge planning.
- Identify problems or abnormal findings, initiate appropriate follow-up, and escalate concerns to the physician or care manager as needed.
- Implement and monitor the plan of care, ensuring care delivery is safe, timely, and evidence-based; maintain accurate and legible documentation consistent with policies and EPIC usage.
- Coordinate patient care across disciplines, incorporating input from nursing, social work, rehab services, and physicians to achieve optimal outcomes.

Requirements
- Active Registered Nurse license in the state of practice (California) or applicable state of assignment
- CPR certification or BLS as required by policy
- Minimum two years of recent nursing experience in the field
- EPIC experience required
- Valid driver’s license and current automobile insurance; must be at least 21 years of age
- BSN is preferred; acute care facility or home care experience preferred
- Ability to work effectively in a hospital setting and handle a diverse patient caseload
