Overview

Responsibilities
- Provide clinical nutrition services to residents in the long-term care setting and to hospital-based patients and other areas as needed
- Perform nutrition assessments, develop and implement individualized care plans, and monitor progress
- Deliver medical nutrition therapy and education to residents, families, and interdisciplinary teams
- Collaborate with physicians, nurses, and foodservice staff; serve as a clinical liaison across care areas
- Ensure compliance with regulatory standards (Joint Commission, NYS Department of Health, CMS) and participate in quality improvement activities
- Document care plans and outcomes, and provide ongoing clinical support to foodservice personnel

Requirements
- Active Registered Dietitian license/registration in New York
- BLS certification
- Master’s degree preferred
- Clinical nutrition experience; long-term care or hospital-based experience preferred
- Ability to work in a long-term care and hospital-adjacent clinical environment
- Availability to work the listed 8-hour day shifts and 40-hour workweek for the 23-week assignment
