Responsibilities
- Conduct comprehensive nutrition assessments for patients/residents identified with nutritional risk or special needs.
- Develop, implement, and reassess individualized nutrition care plans.
- Provide patient and family education on nutrition, diet plans, and therapeutic adjustments.
- Collaborate with an interdisciplinary care team to coordinate nutrition interventions and goals.
- Document all nutrition interventions clearly in the medical record and ensure compliance with policy and standards.
- Apply evidence-based practice and clinical decision-making to deliver high-quality nutrition care.

Requirements
- Active California Registered Dietitian license (or eligibility to obtain) to practice clinically in the listed setting.
- Clinical experience in patient- or resident-centered nutrition care.
- Strong assessment, care-planning, and documentation skills.
- Excellent communication abilities for patient education and interdisciplinary collaboration.
- Ability to work 8-hour day shifts within a travel assignment framework.
- Willingness to travel to the assignment location and adhere to assignment requirements.
