Responsibilities
- Assess residents’ physical therapy needs and develop individualized plans of care in collaboration with the care team.

- Implement evidence-based treatment techniques and modalities appropriate for older adults in a SNF setting.
- Reassess progress, document progress notes, and adjust plans of care to meet patient goals.
- Collaborate with physicians, nursing staff, and therapy assistants; communicate changes in patient status.
- Ensure timely documentation and adherence to facility protocols and regulatory requirements.

Requirements
- Active physical therapy license in New Hampshire (or eligibility to practice per state rules)
- Availability for a 13-week travel assignment in this location
- Experience in skilled nursing facility or geriatric rehabilitation settings preferred
- Strong communication and teamwork skills
- Ability to complete required documentation and comply with regulatory standards
