Responsibilities
- Perform initial evaluations and ongoing reassessments of residents’ physical abilities to identify functional goals and develop a individualized treatment plan.
- Implement evidence-based PT interventions focusing on gait, balance, strength, ROM, functional mobility, and activities of daily living.
- Document treatment sessions clearly in the resident’s medical record and communicate progress to the interdisciplinary team.
- Monitor safety, apply appropriate modalities and therapeutic exercises, and adapt plans based on patient response and goals.
- Educate residents and families on home exercise programs, fall prevention, and transfer/ambulation strategies to support continuity of care after discharge.

Requirements
- Active Physical Therapist license in New Mexico or eligibility to practice in New Mexico
- Ability to work in a skilled nursing facility setting with geriatric rehab needs
- Availability to commit to a 14-week travel assignment with 35 guaranteed hours per week
- Strong clinical assessment, documentation, and communication skills for collaboration with an interdisciplinary team
- Prior SNF or geriatric rehabilitation experience preferred
