Physical Therapist - Skilled Nursing Facility Travel Assignment in Lebanon, New Hampshire. Start date 12/01/2026 for 13 weeks. Weekly pay range $2,675-$2,789 per week. Guaranteed hours 35 per week. Shift 5x8 Days (08:00-16:30). Setting: skilled nursing facility.

Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans
- Implement PT interventions including therapeutic exercise, gait training, balance, and mobility training
- Reassess progress, modify goals as needed, and document outcomes
- Administer modalities and assistive devices as appropriate; monitor safety and response
- Maintain accurate documentation and communicate with an interdisciplinary team
- Educate residents and families on home programs, safety, and prevention strategies; support discharge planning
Requirements
- Active physical therapist license in New Hampshire in good standing
- Doctor of Physical Therapy (DPT) degree
- CPR/BLS certification
- Experience in geriatric rehabilitation or skilled nursing facility setting preferred
- Ability to work in a skilled nursing facility environment and travel as assigned
- Eligible to work in the United States without sponsorship
