Responsibilities
- Perform initial and ongoing PT evaluations for residents and develop individualized plans of care.
- Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, gait, and range of motion.
- Monitor and document patient progress, adjust treatment plans as needed, and communicate progress to the care team.
- Collaborate with nursing, rehabilitation, and medical staff; participate in discharge planning and provide home exercise programs and caregiver education.
- Maintain accurate, timely documentation in the charting system; ensure compliance with regulatory requirements and facility policies.
- Promote patient and family education on safety, fall prevention, and maintenance of gains after discharge.

Requirements
- Active Physical Therapist license or eligibility for licensure in New Hampshire
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Current CPR/BLS certification
- Prior clinical experience in a skilled nursing facility or geriatric setting preferred
- Ability to work effectively in a multidisciplinary team within a skilled nursing facility
- Strong communication and documentation skills
