Responsibilities
- Perform initial evaluations of inpatients and develop individualized plans of care for the hospital setting.
- Deliver PT interventions to support mobility, gait, balance, strength, endurance, and functional activities; adjust treatment plans based on patient progress.
- Document assessments, treatments, progress notes, and discharge planning in the medical record according to facility and program requirements.
- Collaborate with the interdisciplinary care team (nursing, therapy colleagues, physicians, case management) to support safe, efficient patient flow and discharge planning.
- Adhere to infection control, safety, privacy, and clinical guidelines; maintain up-to-date BLS certification and relevant practice standards.

Requirements
- Active New York physical therapy license
- BLS (Basic Life Support) certification
- Prior experience in a skilled nursing facility required
- Availability to work on Saturdays (Saturday-only schedule)
- Ability to work effectively in a hospital/inpatient rehabilitation environment
