Responsibilities
- Plan and conduct segments of rehabilitation treatment programs to restore and improve physical function, coordinating with patients’ goals and department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, contributing to comprehensive care planning.
- Perform evaluations and establish individualized treatment plans; select appropriate modalities and techniques to address assessment findings.
- Provide therapy interventions using approved protocols; develop individualized care and discharge plans and communicate these plans to the relevant team members.
- Maintain accurate, timely clinical documentation; ensure documentation supports quality standards and deadline requirements.
- Supervise assigned assistants and collaborate with the care team to optimize patient outcomes. Document care using the required electronic system (EPIC) as specified.

Requirements
- Doctor of Physical Therapy (DPT) degree; required (only if license obtained on/after January 2016 or foreign equivalent).
- Physical Therapy licensure in all applicable states of practice: Required.
- Current CPR certification; BLS or equivalent per policy.
- Valid Driver’s License; minimum age 21.
- At least two years of recent experience in the field.
- Current nursing license per the state of assignment (as noted in requirements).
- EPIC experience required (documentation and charting in EPIC).
- Ability to work in the listed setting and comply with assignment-specific duties.
