Responsibilities
- Conduct initial and ongoing physical therapy evaluations to establish patient-specific treatment plans.
- Develop and implement individualized therapy plans using evidence-based techniques; adjust based on patient response.
- Provide rehabilitation interventions (therapeutic exercise, gait training, manual therapy, functional mobility training) to restore function.
- Collaborate with the interdisciplinary team; communicate discharge plans and progress to patients, families, and care partners.
- Document comprehensive clinical notes and discharge summaries within established deadlines; supervise assigned therapy assistants.
- Maintain patient safety and adhere to infection control and regulatory requirements; support EPIC documentation as applicable.

Requirements
- Doctor of Physical Therapy (DPT) degree; required if license obtained on or after January 2016; or foreign equivalent
- Physical Therapy licensure in state of practice (California) required
- Two years of Physical Therapy experience preferred
- CPR certification or Basic Life Support (BLS) certification from an approved vendor
- Pelvic floor therapy experience and PRPC or WCS certification or actively pursuing certification
- EPIC experience preferred
- At least two years of recent field experience, current BLS certification, and a current nursing license per the state of assignment
