Responsibilities
- Conduct initial and ongoing occupational therapy assessments for residents.
- Develop and implement individualized treatment plans targeting activities of daily living, mobility, upper-extremity function, and cognition.
- Deliver direct therapy sessions and monitor progress toward functional goals.
- Document evaluations, treatment plans, progress notes, and outcomes in compliance with regulatory and facility requirements.
- Collaborate with an interdisciplinary team (nursing, therapy colleagues, social work) to optimize function and support discharge planning.
- Educate residents and families on treatment goals, therapeutic activities, and home exercise programs; ensure safety and HIPAA compliance.

Requirements
- Active Occupational Therapy license in Washington state or eligibility for WA licensure.
- Ability to work effectively in a skilled nursing facility setting.
- Strong assessment, documentation, and communication skills.
- Availability to work the assigned shift and duration of the contract.
