Responsibilities
- Perform occupational therapy evaluations for residents in a skilled nursing facility and develop individualized treatment plans.
- Implement therapy focusing on activities of daily living, mobility, coordination, and perceptual skills; monitor progress and adjust plans as needed.
- Document assessments, goals, progress notes, and discharge planning in the electronic medical record.
- Collaborate with nursing staff, therapy assistants, and social work to coordinate comprehensive resident care.
- Educate residents and families on home exercise programs, safety strategies, and caregiver instruction.
- Support discharge planning and transition processes to promote continuity of care and appropriate follow-up.

Requirements
- Active Occupational Therapist license in New Mexico or eligible for licensure.
- Ability to work effectively in a skilled nursing facility environment.
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team.
- Experience in geriatric or SNF settings preferred, but not required.
- Willingness to travel for a travel assignment and adapt to varied resident needs.
