Responsibilities
- Perform comprehensive occupational therapy evaluations for residents in a SNF setting.
- Develop, implement, and adjust individualized treatment plans to improve activities of daily living, mobility, cognition, and functional independence.
- Provide direct therapy sessions and therapeutic activities, documenting progress and outcomes.
- Collaborate with an interdisciplinary team to modify goals, discharge plans, and care plans as needed.
- Educate residents and family members on home exercise programs and strategies to promote safety and independence.
- Ensure timely charting, compliance with regulations, and accurate reporting of patient status and progress.

Requirements
- Active occupational therapy license in good standing (state of Washington) or eligibility for licensure
- Ability to work in a skilled nursing facility environment
- Willingness to participate in a 13-week travel assignment
- Strong documentation, communication, and collaboration skills
