Responsibilities
- Assess patient condition and nursing needs; plan and implement nursing care under supervision of a registered nurse and director of nursing.
- Administer medications following the eight medication rights to reduce the potential for errors.
- Ensure continuity of care between shifts by providing direct patient care and supervising CNAs and supportive staff.
- Document changes in patient condition and nursing care on progress notes and care plans; maintain accurate records.
- Perform waived (point-of-care) testing per policy; interpret results and take appropriate action.
- Notify physicians of changes in patient condition and follow through until appropriate action is taken; participate in thorough shift handoffs and care coordination.

Requirements
- Active California LVN/LPN license
- Current Basic Life Support (BLS) certification
- Ability to read and communicate effectively in English
- Basic computer knowledge; Electronic Health Record experience preferred
- Additional languages preferred
- Ability to work night shifts in a hospital setting
