Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
- Contribute to developing an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve residents and families in the process.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals to ensure comprehensive care.
- Maintain current knowledge and competency in nursing practice; participate in the mentoring and orientation of staff; support safe and cost-conscious care delivery.
- Demonstrate leadership in the clinical setting under the direction of the RN; adhere to safety, infection control, and documentation standards; work effectively with other nursing staff, physicians, and ancillary team members.

Requirements
- Active New York LPN licensure
- BLS certification (American Heart Association or RQI)
- Weekend and holiday availability required
- Minimum 1 year of experience serving in an LPN capacity at a long-term care facility
- Ability to work the Days shift and collaborate with a multidisciplinary team
