Responsibilities
- Provide direct and indirect skilled nursing care to adult and geriatric residents on the assigned unit under the direction of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to the development of an age-appropriate and culturally relevant plan of care; implement interventions and involve residents and families in goal setting.
- Monitor resident status, adjust care plans as needed, and refer to appropriate team members to address all aspects of care.
- Maintain current knowledge of nursing practice; participate in orientation and inservice activities; support a safe, collaborative care environment.
- Demonstrate leadership within the professional practice setting, and collaborate with physicians, nurses, and ancillary staff to promote quality, safe care; handle exposure to bloodborne pathogens as expected in routine practice.

Requirements
- Current New York Licensed Practical Nurse (LPN) license
- BLS certification (AHA or RQI acceptable)
- Minimum 1 year of LPN experience, specifically in a long-term care facility
- Ability to provide care to adult and geriatric residents
- Ability to work as part of an interdisciplinary team under RN supervision
- Prior LTC experience strongly preferred
