Transition Youth Specialist

Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a dedicated and qualified professional to provide community-based services to youth ages 14–18. This role is focused on supporting participants as they develop and strengthen independent living skills and employment readiness.

Key Responsibilities:

Conduct initial and ongoing assessments of participants’ independent living skills, vocational strengths, and interests
Develop individualized service plans aimed at achieving participants' goals in areas such as education, employment, and daily living
Provide hands-on support and coaching to assist youth in obtaining and maintaining employment
Facilitate access to resources and create learning opportunities that promote self-sufficiency
Collaborate with service providers, families, and systems of care to ensure coordinated support

Ideal Candidate Will Have:

Experience working with transition-age youth
Strong understanding of independent living and vocational support services
Ability to engage young people in a strengths-based and goal-oriented manner
Excellent organizational and communication skills
A commitment to empowering young people toward greater independence

Qualifications: Requirements

Must be willing to transport youth in personal vehicle as needed.
BA/AA degree required.
