Transition Advisor (Part-Time)

Transition Advisor is a part-time (5-10 hours/week) hourly role earning $19/hour

What you will do:

• Manage caseload with required one-to-one sessions 
• Implement and modify curriculum for classes 
• Facilitate and/or Co-Facilitate classes based on setting goals, navigating resources, and increasing independence 
• Provide personalized support that is aligned with programmatic goals and outcomes 
• Complete monthly evaluations for each individual on caseload 
• Maintain and update activities and resources focused on transition-aged individuals

What we expect of you:

• Bachelors degree in social work, special education, non-profit management, or related field preferred 
• Two or more years of experience working with transition-age individuals with intellectual and/or developmental disabilities preferred

• Willingness and ability to learn various technology platforms and effectively teach participants how to use them 
• General knowledge of special education services and post-high school options
• Strong understanding of effective ways to support teens and young adults with disabilities
• Strong relationship-building skills with individuals, families, and colleagues
• Experience leading and guiding teens and young adults in both individual and group settings
• Confidence facilitating curriculum for both small and large groups
• Resourcefulness in navigating technology, applications, and community-based resources
