T1D Support Guide

We are seeking a dedicated and compassionate Type 1 Diabetes (T1D) Support Guide to join our healthcare team. As a T1D Support Guide, you will play a crucial role in supporting our participants diagnosed with T1D. Your primary responsibilities will be to support comprehensive care coordination and patient education through our virtual program to promote optimal health outcomes for our participants. You will also provide peer support mentoring and serve as a trusted point of contact for our participants. 

Our T1D Support Guides, or Guides, are intimately familiar with T1D and understands the daily challenges of managing it. As a remote organization, care is provided to participants via phone, SMS, email and video. Our hours of operation are 8:00am - 8:00pm EST, Monday-Fridays. This is a full-time position, 40 hours per week. This role requires a consistent Wednesday & Thursday schedule of 11am-8pm EST. Monday, Tuesday & Friday, exact hours are flexible and can be determined upon hire, with 9 hours per day, including a 1 hour lunch break. To meet our patient’s needs, the schedule will need to be consistent once set. A private, quiet, home office space is required for patient appointments.  

This role is open to candidate residing anywhere within the Unites States who are legally authorized to work in the U.S. on a full time basis, now and in the future, without employer sponsorship. 

• High level knowledge and understanding of living with Type 1 Diabetes, either personally or with a close family member required.
• Minimum of 2 years experience in healthcare required, with 6 months+ supporting individuals living with T1D preferred.
• Experience in telehealth or remote work required.
• High school diploma or equivalent required.
• Must be fluent in English, both verbal and written.
• Proficiency in using electronic health records (EHR) and other healthcare software systems required.
• Dual language, Spanish/English, preferred.
• Ability to work independently and as part of a collaborative team.
• Ability to read, synthesize and understand patient charts in preparation for visits.
• Proficient in interpersonal communication across with people from diverse backgrounds and experiences, with a strong ability to connect and understand others effectively.
• Ability to listen actively, communicate with empathy and gather information in a respectful manner. 
• Demonstrates curiosity, humility, and respect when working with people whose backgrounds, experiences, beliefs, or circumstances differ from your own. 
• Communicates calmly and compassionately with participants experiencing stress or crisis, providing reassurance, clear guidance, and emotional support.
• Uses sound judgment to recognize when patient needs require timely escalation or additional support, and follows established protocols to ensure safety and continuity of care.
• Ability to effectively communicate information to participants, medical professionals, clinic employees and other community organizations over the phone or virtually.
• Ability to communicate complex health information in clear, accessible, and understandable language and adapt communication to different levels of health literacy, technology comfort, and individual needs. 
• Takes initiative to proactively identify patterns, trends and opportunities for improvement.

• Guide participants who live with Type 1 Diabetes through the Blue Circle Program.
• Support and build relationships with people living with T1D to achieve better health outcomes. Guides provide support from a non-clinical perspective. 
• Educate participants about the Blue Circle Health T1D program. 
• Empower the people we serve to advocate for their health and fully understand the resources available to them.
• Participate in coordinating care for participants: this may include appointment tracking, facilitating internal and external services, assisting in identifying external continuity of care, and proactive participant reach-out.  
• Support participants in preparing for upcoming clinical visits, including setting up Zoom with participants and helping participants understand the technology used by our virtual program.
• Monitor participants' progress, and work with the Care Team to adjust care plans as needed to achieve desired health goals.
• Assist in setup and syncing of diabetes devices to respective online portals to ensure smooth provision of care with the medical team
• Owns the patient communication center in our EHR to ensure patient communications are addressed and responded to appropriately. 
• Maintain accurate, timely and up-to-date documentation of patient interactions and care plans in electronic health records (EHR) systems.
• Coordinate with the clinical care team by providing useful information to assist other service lines in resolving medical and non-medical needs.
• Work closely with the clinical care team to identify situations in which medical management may be needed and encourage participants to follow-up with this care.
• Contribute to other priorities and projects as needed.

Compensation: 
$24-$29/hour. This range reflects the good faith estimate of the compensation we reasonably expect to pay for this position. Individual compensation decisions are based on job related factors, including skills, experience, and internal equity.
