Job Details: Job Location: Newton PV - Newton, KS, Salary Range: Undisclosed, POSITION GOAL:  Peer Support (PS) services are individual centered services with a rehabilitation and recovery focus aimed at helping build recovery capacity for persons new to recovery by connecting them to naturally occurring resources in the community, assisting in reduction of barriers to fully engaging in recovery, and providing support in skill development for maintaining a recovery lifestyle.

CPA 1:  PEER SUPPORT COMPONENTS *

Work with assigned counselor to provide peer support services to assigned persons receiving services with the goal of helping the newly recovering person to transition into a recovery lifestyle.
Provide group and educational presentations as required.
Provide transportation for clients to community based providers, which might include appointments to court hearings, doctor’s offices, parole/probation officers, etc.
Assist patients individually and in group settings to identify and work toward long term recovery goals.
Work with persons receiving services to improve access to housing, food, education, job training, transportation, etc.
Facilitate connections and linkages for persons receiving services and the recovery community.
Willing to share personal recovery experiences, normalizing that addiction is a chronic disease and that recovery is possible.
Provide education to family members, individuals, and community members around addiction and recovery.
Help the service participant to develop a network for information and support from others who have been through similar experiences.
Assist service participants with regaining the ability to make independent choices and to take a proactive role in treatment, including discussing questions or concerns about treatment approaches with their treating clinician.

Assist the service participant with identifying and effective responding to or avoiding identified precursors or triggers that result in functional impairments and/or relapse.

CPA 2:  ADMINISTRATIVE RESPONSIBILITIES *

Accurately complete Etime on a daily basis.
Communicate with community partners only after obtaining proper releases of information authorization according to 42 CFR Part 2.
Assist licensed addiction counselors with the development of discharge/transition plans with patients, assisting with connections to community resources as appropriate.
Participate in case staffing conferences when requested by your supervisor.
Attend and regularly participate in clinical and administrative supervision.
Maintain responsibility for organizing a schedule and establishing priorities, according to existing patient and agency needs.
Assist clinical and administrative staff with KDADS compliance and/or funding source audits.      
Assist in the process of maintaining patient charts in a site visit ready manner.
Able to collaborate with a team to support the patient throughout their continuum of care. 
Maintain adequate and timely paperwork tasks and documentation in the clinical record of treatment interventions and other activities related to service participant care, as defined by Prairie View standards.
Complete schedules within 24 hours of the service provided.
Participate as a member of Prairie View staff in structured meetings.
Participate in continuing education activities and required in services (such as Infection Control, etc.) as requested, to enhance and improve job performance.
Perform other job related functions as directed by your immediate supervisor.    

CPA 3:  PRODUCTIVITY EXPECTATIONS *

Meet patient encounter expectations of 24 per week.
Ensure contacts with service participants are goal directed, outcome focused, and meaningful.

CPA 4: QUALITY ASSURANCE AND PERFORMANCE IMPROVEMENT (QAPI) *

Participate in organizational QAPI activities.  Be familiar with QAPI Initiatives and goals in work area.  Annually review the Quality Assurance & Performance Improvement Plan.

CPA 5:  CONFIDENTIALITY *

Promote professionalism by discussing personal or service participant information in a private area.
Maintain person receiving services and program confidentiality in all aspects of position as required by federal laws.
Be familiar with federal confidentiality requirements according to 42 CFR Part 2 and how these differ from HIPPA.

POSITION DESCRIPTION

This position description is a temporary management guide tool and is subject to change.Qualifications: POSITION QUALIFICATIONS:

Minimum Education:  High school diploma or equivalent; documentation of certification as a Kansas Certified Peer Mentor.
Minimum Experience:  Minimum of one year in stable recovery, defined as actively working in a recovery program (e.g., twelve step, church/spiritual, other recovery support group).
Provider must self identify as a present or former primary client of Prairie View services.  For Prairie View Substance Use Disorder Services alumni, you will need to be out of Prairie View treatment services for at least one year.
Must be at least 18 years of age.
Must have good reading, writing, and communication skills – listening, verbal, and written.
Must be able and willing to be flexible, a self starter, punctual, and have the ability to maintain a schedule
Must have a valid Kansas driver’s license, be insurable through Prairie View insurance company, and be willing to provide transportation to patients as needed.
Must have ability to maintain professional boundaries with persons receiving services.
Must be proficient at basic keyboarding/word processing skills or skills with an electronic health record.
Must be able to pass Kansas Bureau of Investigation Screen and the Central Registry Screen for both adults and children.
Regular and predictable attendance.
Knowledge of the recovery process, resources, and ability to engage persons in their own individualized recovery plans and goals.
