Summary:Supports Bridgewell's mission by overseeing crisis and behavioral support needs of individuals receiving supports through DDS. Works in conjunction with other team members and services as needed to develop personal support plans, triage emergency calls including mobile response, clinical consultation, liaison to hospitals, referral to outpatient services and specialized assessments, and on-call crisis response for individuals referred to START by DDS. Develops and monitors behavior support plans for individuals within Bridgewell's residential programs.

Schedule: Part-time 25 hours per week with flexibility (Monday-Friday 9:00am-5:00pm) On-Call rotation required

Essential Duties & Responsibilities:

1. Develop personal support plans outlining crisis management strategies for individuals referred to START by DDS.
2. Facilitate communication among team members related to the crisis planning process to assist with service needs/requests.
3. Assist in identifying necessary preventative resources/services.
4. Participate in related clinical/team meetings (i.e., weekly triage meeting, clinical meetings, etc).
5. Provide in-home training/support/assessment for individuals pertaining to implementation and documentation of crisis supports, and providing training and education to primary care providers, crisis teams, hospitals, etc.
6. Access Sovner/START affiliated consultants as appropriate and attend relevant consultations for clients on caseload.
7. Maintain updated client records, and maintain documentation of all client related contact.
8. Assist with crisis response and disposition process as appropriate, i.e. participate in triage and on-call systems, complete assessments for clients in crisis, assist with psychiatric hospital bed search, referral to START Respite/affiliated respite, attend intake/discharge meetings.
9. Other tasks as designated by supervisor.

Required Education/Experience:

1. BA in human services field and 3 years of experience working with dually diagnosed individuals (ID/DD and psychiatric diagnosis) and/or relevant experience.

Required Skills/Knowledge:

1. Strong leadership and problem solving skills
2. Strong clinical skills
3. Demonstrated knowledge of behavioral concepts and techniques, and crisis intervention strategies
4. Demonstrated ability to develop and foster positive relationships with funding sources and service providers
5. Excellent written and oral communication skills.
6. Maintain CPR/1st Aid certification

Physical Demands:

1. Employee is required to drive a motor vehicle.
2. Must have a normal range of eyesight with or without corrective lenses.
3. Must have a normal range of hearing with or without corrective equipment
