Provide social work services to patients in acute care, maternal child care, emergency department, home health/ hospice setting for patients with acute, chronic, or terminal illness and their families. Services to patient and families include: identification of psychosocial needs, develops, and implements plan of care from the strengths perspective responsive to patient/family concerns, health care status, available resources, age specific problems and clinical impressions; discharge planning, crisis intervention, information and referral, protective services, patient family education, collaboration with the multidisciplinary care team, and building connections with community resources.

QualificationsJOB QUALIFICATIONS:

Experience:

Minimum Preferred Experience: one year experience
Preferred Experience: 2 years experience in medical field

Education: 

Minimum Required Education: Bachelor degree Social Work 
Preferred Education: Masters degree Social Work 

Licenses:

Minimum Required Licenses: Licensed Bachelor of Social Work
Preferred Licenses: Licensed Master Social Work; Licensed Specialist Clinical Social Worker
