Job Summary

SIGN ON BONUS AVAILABLE for eligible Non-MGB employees:
$3,000 for LCSW and $10,000 for LICSW

Starting rates:
LCSW: $32.80/hr
LICSW: $44.34/hr

The Social Worker is an integral part of the Bridge Clinic, a low-barrier, transitional outpatient clinic for addiction care at MGH. This position is a critical member of a collaborative care team that includes a resource specialist, recovery coach, physicians, nurse practitioners, psychologists, nurses, and patient service coordinators. This position will both provide direct clinical care and associated team-based care coordination. The primary clinical responsibilities will be to provide psychosocial assessment, treatment planning, brief evidence-based treatment, including motivational intervention, CBT, and psychoeducation, for Bridge Clinic patients, particularly those with co-occurring mental health and substance use disorders. This role will have responsibility for rapid assessment, crisis management and team coordination for patients with acute presentations, as well as responsibility to assist with care planning and coordination for patients with severe dual diagnosis conditions. In addition, this role will collaborate with all team members to develop and support transition plans for these patients within all levels of the addiction treatment care and mental health continuum.

PRINCIPAL DUTIES AND RESPONSIBILITIES
Intake Coordination:
o Assess patients who are referred to Bridge Clinic from inpatient, ED, and community settings with complex mental health and addiction needs to aid with evaluation, treatment planning, and linkage to most appropriate care placement.
o Assist in providing brief assessments for urgent or unscheduled walk-in patients to ascertain specific mental health care needs, in collaboration with nurses or providers, and assist with developing effective care plans as needed.

Clinical Care:
o Provide trauma-informed, short-term, outpatient individual and/or group counseling, utilizing psychoeducation, motivational interviewing, harm reduction, and cognitive behavioral therapy techniques to assist patients with change readiness, recovery stabilization and wellness goals.
o Provide clinical support to complex patients on transition planning as they may need clinically informed transfer of care utilizing motivational interviewing and trauma-based perspectives.
o Provide crisis intervention and de-escalation as needed for patients who present for care with acute mental health needs utilizing trauma informed approaches.

Care Coordination:
• Function effectively as a member of a multidisciplinary, integrated medical and psychiatric health care team. Provide consultation to staff across role groups to help support understanding and effective intervention with patients.
o Work collaboratively with patients and Bridge Clinic staff to assist with patients’ transition to long term community-based addiction treatment as appropriate. Ensure integration of care and effective utilization of resources through review of documentation, discussion with providers, and collaboration with patients.
o Support patients’ transfer to all levels of care and interactions with other systems (DCF, probation, incarceration etc.) through the provision of clinical information and documentation to other facilities and systems.

Qualifications

The position requires a high degree of flexibility, independence, and willingness to participate in multiple activities that provide support to both patients and caregivers.

The position requires strong communication skills and the ability to converse comfortably with patients and their caregivers, as well as with clinical and administrative staff, and to communicate with care team effectively via documentation in medical records   

Social Worker I:

Clinical experience in medical setting 0-1 year required

Licensed Clinical Social Worker [LCSW - State License] required

Social Worker II:

Clinical Experience in a medical setting 2-3 years required

Licensed Independent Clinical Social Worker [LICSW - State License] required ​

• At least 2-3 years of work experience in the mental health care field, especially with patients in acute settings, e.g. emergency mental health care, inpatient psychiatric treatment units, or short-term mental health observation units preferred. Focused training in these areas may be substituted.

• At least 2-3 years of work experience in caring for patients with substance use disorders strongly preferred.  Significant training experience in substance use disorders may be substituted.

• Excellent skills in rapid assessment, de-escalation and crisis intervention.

• Excellent interpersonal, collaborative, customer service and advocacy skills

• Excellent written and verbal communication skills

• Excellent organizational and time management skills; ability to prioritize and multitask in a fast-paced environment

• Excellent judgment and ability to work independently in a unique position

• Flexibility and resourcefulness as well as strong assessment and problem-solving skills

• Strong computer skills

• Familiarity with addiction treatment, dual diagnosis treatment, systems of care, and community resources

• May provide clinical supervision based on department need (MSW students and/or social work staff).

• Seeks opportunities in the department and hospital or may be asked by SW leadership to participate in initiatives, projects, quality reviews, and committees and participates in these at an entity and Enterprise level.

• Creates content for skill-based groups.

• Promotes timely and concise collaboration among all disciplines and outside stakeholders to enhance patient care (including but not limited to, Ethics Committee, SDoH Committee, Office of General Counsel, Case Management department, etc.).
