JOB DESCRIPTION

Summary/ Objective

East Side House (ESH) is a community resource in the Bronx and Northern Manhattan. We believe education is the key that enables all people to create economic and civic opportunities for themselves, their families, and their community.  ESH’s Social Services Programs align with our mission by providing high quality programs and services within community programs and school partnerships.  

The Social Worker provides social work and supportive services to participants in Expectant Families and Older Adult Programs. Working collaboratively with program staff, the Social Worker helps individuals and families address immediate needs, strengthen stability and well-being, and connect to appropriate resources. Services include needs assessment, emergency asssisance, short-term supportive counseling, crises support, psychoeducational workshops, referrals, and coordination of care. 

Essential Functions and Responsibilities

Under the supervision of the Program Director, the Social Worker, will be responsible for but not limited to the following:

Conduct initial and ongoing needs assessments and develop appropriate support referral, and follow-up plans. 
Coordinate emergency assistance and basic-needs support, including required documentation and follow-up. 
Provide short-term supportive counseling, crises support, safety planning, and appropriate referrals within the scope of the role. 
Connect participants and families to internal and community resources and public benefits, including housing, food, healthcare, behaviorial health, childcare, legal, financial, workforce, and other family supports. 
Plan and facilitate psychoeducational workshops, groups, and events based on participants needs, including wellness, parenting preparation, stress management, healthy relationships, financial capability, and resource navigation. 
Collaborate with Home Visitors and other program staff to identify  participant needs, coordinate services, and address barriers to participant goal attainment. 
Maintain relationships with community partners and connect participants to relevant East Side House programs and services. 
Maintain timely, accurate, and confidential documentation of assessments, services, emergency assistance,  referral, outreach, workshops, and participant outcomes. 
Fulfill mandated reporter responsibilities and respond to participant safety and concerns in accordance with applicable law, agency policies, and program protocols. 
Participate in case consultation, program planning, professional development, Social Services Department, and organization-wide events activities as assigned.

Qualifications: Qualifications

To perform this position successfully, the Social Worker must be able to fulfill all the elements of the job description to the highest standard and be able to work well with colleagues in a high energy and collaborative work environment.  Additional qualifications include:

Master of Social Work (MSW); social work licensure not required
Minimum of 2-3 years of relevant experience in social services, case management, family support, community-based services, or related setting. 
Experience working with expectant families, families with young children, older adults, and /or diverse community populations preferred. 
Demonstrated experience with needs assessment, resource navigation, referrals, advocacy, crises support, and service coordination. 
Knowledge of New York City public benefits, emergency assistance, healthcare, behavioral health, housing, food security, and other community-based resources.
Experience developing or facilitating psychoeducational workshops or groups preferred. 
Knowledge of mandated reporting, participant safety, confidentiality, professional boundaties, and documentation requirements. 
Strong written and verbal communication skills and proficiency with electronic case-management/data systems and Microsoft Office.
Billingual English/Spanish strongly preferred. Commitment to demonstrating ESH’s organizational values of Excellence, Integrity, Compassion, Empathy, and Community
SIFI Certification strongly preferred with at least one year’s experience supervising social work interns
Knowledge of the network of social services available to parents and families in New York
Bilingual in English and Spanish

Expected Work Schedule
The work schedule expected for this position is: 8:00am – 4:00pm Monday - Friday, as aligned to the needs of the program/service.   The schedule may be adjusted based on ESH’s operational needs and funder requirements, with notice provided whenever possible.
