How you'll make an impact in this role

• Guide seamless patient transitions by creating individual care plans across the full spectrum of recovery—blending clinical needs directly with patient and family values.

• Resolve critical psychosocial barriers as a subject-matter expert in crisis intervention, legal support, substance abuse, and adjustment needs to protect patient well-being.

• Advocate for essential resources by proactively identifying community partnerships and systematically removing obstacles to post-discharge care.

• Partner with interdisciplinary teams through continuous communication to evaluate health plans, optimize medical record documentation, and ensure every patient safe, high-quality care transitions.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• One or more of the following required:
  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
