How you'll make an impact in this role

• Design personalized care plans: Partner with patients and families to build holistic continuum-of-care strategies that align medical goals with their unique personal values and preferences.

• Deliver expert psychosocial interventions: Evaluate and support complex patient needs—including crisis management, substance abuse, and legal or adjustment challenges—to guide families through difficult transitions.

• Break down care barriers: Advocate relentlessly for necessary community resources and eliminate social determinants of health to ensure equitable, uninterrupted care.

• Guide post-discharge transition planning: Consult with multidisciplinary teams to ensure precise medical documentation, recommend appropriate care levels, and secure safe post-acute services.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• One or more of the following required:
  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
