Job DetailsJob Location: SHC of Bluffton - Okatie, SC 29909Sprenger Health Care is a third generation, leading developer, owner and operator of senior living communities and services in Ohio and South Carolina. Qualifications: 2 years of experience in long term care, licensed social worker or bachelor’s degree preferred, must possess excellent communication and organizational skills

Essential Job Functions:

Develop one-to-one relationships with residents and families.
Establish relationships with area hospice agencies.
Act as a liaison with social, health and community agencies.
Coordinate with Activity Department to meet socialization needs of the residents.
Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
Assist in admissions and discharge process as needed.
