Job Summary

A Social Services Coordinator to help senior citizens maintain their independence and the dignity to age in place within the community. The Social Services Coordinator supports our senior residents by answering questions, addressing concerns and assisting with linkages to basic needs such as meals, housekeeping, transportation, personal services, financial assistance, counseling and other social supports.

This is a part-time (20 hours/week) role. A consistent schedule supporting both business and resident needs is required Monday - Friday, between the hours of 7:30am and 6:00pm. 

Key Responsibilities:

Contribute to caring and positive working and living environments for all employees and residents
Build rapport with residents while maintaining appropriate professional boundaries to help them age in place successfully.
Support resident autonomy and dignity through the identification, linkage and/or provision of services that help individuals live as safely, independently, and comfortably as possible.
Identify resident needs and preferences through an assessment process ensuring 95% of residents have been contacted for assessment or reassessment in the last 12 months. 
Identify, develop and facilitate/attend 90% of resident activities per month.
Facilitate or arrange resident support groups which help residents to process feelings and develop new relationships in the community.
Assist residents with acquiring In Home Support Services where appropriate and available.
Help residents adhere to a care plan and address behaviors that negatively impact community living, including conflict resolution between residents or resident and staff. When appropriate, communicate with family members.
Within parameters of HUD and other regulatory requirements, advocate for individual residents without compromising their privacy or the rights of others.
Model and promote courteous, respectful, and non-confrontational communication skills.
Educate peers and others about the special needs of the resident population.
Other duties as assigned

Reporting + Organizational Relationships
This position will report directly to the Associate Director of Resident Services and does not have any supervisory responsibilities.

Compensation & Classification
CCH is committed to paying competitive wages. The salary range for this position is $26.39 - $31.36 in California and $23.01 - $29.23 outside of California. Based on CCH’s compensation philosophy, the majority of new hires are brought in at the minimum of the salary range. CCH has a competitive benefits package with health, dental, vision, and life insurance, 8% of an employer contribution for retirement, and more. This is a part-time, non-exempt position. This position is based at Hatler May Village, located at 2842 Vickers Drive, Colorado Springs, CO 80918.

Qualifications: Qualifications and Requirements

Highschool Diploma or GED required.
Bachelor's degree (B.A.) in social work, psychology or related field strongly preferred.
Professional Service Coordinator (PSC) designation preferred.
Experience in an affordable housing environment preferred.
Demonstrated ability to serve with integrity, professionalism, and confidentiality.
Demonstrated working knowledge of supportive services and other community resources
Demonstrated ability to advocate, organize, problem-solve, and provide results for the residents served.
Advanced interpersonal and communication skills (both written and oral)
Strong leadership skills with a proven ability to motivate others and deliver constructive feedback tactfully.
Strong analytical and problem-solving skills.
Ability to use mediation and conflict resolution techniques
Excellent organizational skills, attention to detail, prioritization and time management skills with a proven ability to meet deadlines.
Proficient with a Microsoft Office 365 platform and ability to quickly learn the organization’s other systems.
