The RN Field Supervisor is responsible for assessing new patients, creating patient-centered plans of care, and documenting progress. Additionally, they provide supervision and training for PCA/HHA staff and participate in an on-call rotation.
Job Description
Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor!
Schedule: 1-2 days per week
Duties & Responsibilities:
- Provide assessment of newly referred patients and assess health needs in home care setting
- Work with patient to create a patient centered plan of care
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
- Provide supervision and training of PCA/HHA staff
- Assist with the on-call rotation (stipend provided)
Qualifications:
- NYS Licensed Registered Professional Nurse
- Minimum of Associate's Degree required
- One year Nursing experience required
- Home care experience a plus

These combined services can ease the stress on families worrying about loved ones living alone. Services are delivered in the home, helping to provide more independence for loved ones.
