RN, Case Manager UM

Job Details: Job Location: Hospital - Hinesville, GA, Salary Range: Undisclosed, JOB SUMMARY

This position is for a Case Manager whose responsibilities includes assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating inter-disciplinary care activities; assists with Swing Bed Program: and coordinates the continued care of transitioned patients.  The position reports to the Chief Nursing Officer.
Qualifications: JOB QUALIFICATIONS

Minimum level of Education: Education level equivalent to completion of an associate degree in nursing.
Formal Training: Clinical competence in nursing preferred.
Licensure, Certifications & Registration: Current license to practice professional nursing in the state of Georgia Current license to practice professional nursing in the state of Georgia; BLS Provider certification required.  Instructor certifications do not replace provider certification requirements.
Work Experience:  Basic technical nursing skills. Experience in chart auditing.  Experience in interpreting federal and state regulations and accreditation standards governing patient medical necessity and continuity.  Basic typing and computer skills.
