About the Role:ComForCare Home Health Care in Arlington, VA is looking for a dedicated and compassionate RN Case Manager (Assessment Only) to join our growing team. Responsibilities:

• Conduct comprehensive in-home patient assessments to determine care needs and eligibility for home health services
• Complete OASIS assessments, start-of-care evaluations, and recertification assessments in compliance with regulatory standards
• Collaborate with physicians, care teams, and family members to develop and communicate individualized care plans
• Document clinical findings accurately and in a timely manner using electronic health records (EHR)
• Ensure compliance with Medicare, Medicaid, and Joint Commission guidelines during all assessment activities
• Communicate patient status changes and clinical concerns to the appropriate care team members
• Maintain a flexible schedule to accommodate patient and agency assessment needs across the Arlington, VA service area

Requirements:

• Active and unrestricted RN license in the state of Virginia
• Minimum of 1-2 years of home health or clinical nursing experience preferred
• Proficiency with OASIS documentation and Medicare home health regulations
• Strong clinical assessment and critical thinking skills
• Excellent communication, time management, and organizational abilities
• Reliable transportation and valid driver's license for home visit travel
• Comfortable working independently in a field-based, patient-facing environment
