RFI - Project DC Homeless Response Action Plan (HRAP)

Background
Close to Home seeks an Implementation Project Manager to support implementation of the Project DC 
Homeless Response Action Plan (HRAP), a community plan developed by homeless service providers and 
system partners.

Project DC, led by the San Antonio Housing Trust (SAHT), is an effort to create a shared, communitydriven vision for a Cattleman District that is safe, welcoming and vibrant for all. A primary component of Project DC is the Homeless Response Action Plan (HRAP) – to end street sleeping in the Cattleman Square area through coordinated outreach, housing-focused service delivery, multidisciplinary collaboration, and strengthened community partnerships before redevelopment occurs in this historic area of San Antonio. The HRAP's central goal is to permanently house the 112–169 people currently experiencing unsheltered homelessness in Cattleman Square within seven months of initiative launch. 
Close to Home serves as the Coordinating Body for the local Continuum of Care (Alliance to House Everyone) and the Project DC HRAP. Close to Home will support governance, coordinate implementation partners, monitor initiative progress, problem-solve identified barriers, and help align the work around shared goals and measurable outcomes to end street sleeping in Cattleman Square.

The HRAP brings together existing community resources through a coordinated implementation 
framework designed to accelerate housing placements in Cattleman District. The HRAP serves as a 
demonstration initiative within the broader San Antonio and Bexar County Homeless Response System. 
Implementation will occur through and alongside the existing Alliance to House Everyone infrastructure 
and governance. Where appropriate, training opportunities designed for HRAP implementation may 
include broader system partners. Lessons learned through implementation will inform the broader 
Homeless Response System’s future work, including: operational practices, housing-focused 
coordination, performance monitoring, and community-wide crisis response.

Purpose

Close to Home is seeking an experienced Implementation Project Manager to help coordinate implementation of the HRAP during its first year, beginning with the current pre-implementation phase.

The Implementation Project Manager will work collaboratively with Close to Home, SAHT, implementation partners, Alliance governance bodies, and community partners to maintain project momentum, organize implementation activities, maintain strong partner relationships, coordinate meetings and follow-up, track milestones, support communications and partner onboarding, identify implementation barriers, and help ensure successful execution of the HRAP.

The Implementation Project Manager will also help connect HRAP implementation to the existing 
Homeless Response System by:

• Reporting progress to CoC governance,
• Preparing implementation partners for performance monitoring,
• Informing CoC and external community communications, and
• Integrating newly funded HRAP projects and service provider

Ultimately, this coordination should support implementation partners in successfully moving people experiencing unsheltered homelessness in Cattleman Square to permanent housing.

This Request for Information is intended to identify an individual consultant with the experience, skills, and approach necessary to support a collaborative community initiative of this scale. Close to Home is seeking one individual to fill this role, rather than sharing responsibilities across a team of multiple people.

The selected Implementation Project Manager will report to Eboni Jett, Close to Home Chief Strategy 
Officer, and will be engaged as an independent contractor (1099) of Close to Home. This is not a W-2 
employee position.

What Success Looks Like

At the conclusion of the engagement, successful implementation will mean that the HRAP has moved from pre-implementation into an organized, measurable implementation structure that is integrated with the existing San Antonio and Bexar County Homeless Response System and Alliance governance. Anticipated outcomes include:

• A clear and actively managed HRAP implementation structure with defined responsibilities, timelines, milestones, dependencies, and accountability across implementation partners.
• People experiencing unsheltered homelessness in Cattleman Square are successfully and permanently housed through the housing pathways, coordinated services, and individualized strategies outlined in the HRAP, with supports in place to promote long-term housing stability (initiative goal: 84-90% housing retention)
• Strong coordination between Close to Home, SAHT, homeless response providers, funded implementation partners, and other community partners contributing to HRAP.
• An operational governance structure in which the SAHT Public Facility Corporation Board and the Close to Home Homeless Strategic Advisory Board provide strategic guidance and oversight, while the Implementation Team and Core Team function as the working bodies coordinating day-to-day implementation and housing placement progress.
• Clear processes for incorporating newly funded HRAP projects and service providers into implementation, coordination, governance, Homelink (the local coordinated entry process), performance monitoring, HMIS and community reporting.
• A defined set of HRAP implementation and outcome measures that align, where appropriate, with existing system performance monitoring rather than creating a separate reporting structure.
• Consistent and transparent communication with homeless response providers, Alliance partners, funders, and the broader community regarding implementation progress, lessons learned, emerging needs, and opportunities for partnership.
• Documented lessons from Cattleman Square that inform improvements to the broader Homeless Response System, including outreach coordination, housing-focused practices, governance, performance monitoring, and future place-based responses.
• A sustainable implementation structure that can continue beyond the initial Implementation Project Manager engagement, with clear ownership of ongoing responsibilities by Close to Home, SAHT, Alliance governance bodies, and implementation partners.

Anticipated Partnership Responsibilities

The Implementation Project Manager will work collaboratively with Close to Home to support 
implementation activities, which may include:

• Coordinate implementation of the Project DC HRAP and maintain the master implementation work plan.
• Track milestones, action items, risks, dependencies, decisions, and implementation barriers.
• Develop and nurture relevant community relationships and facilitate collaboration among implementation partners.
• Support coordination across outreach, housing, healthcare, behavioral health, and other implementation partners to advance people experiencing unsheltered homelessness in Cattleman Square through the housing pathways and individualized strategies outlined in the HRAP, identifying and elevating barriers that delay or prevent progress toward permanent housing.
• Provide regular verbal reports for relevant Committees and Work Groups, including the Cattleman Square Advisory Board
• Maintain visibility into progress toward permanent housing outcomes and work with implementation partners to identify operational, resource, policy, or capacity barriers requiring resolution through the appropriate implementation or Alliance governance structure.
• Coordinate implementation meetings and follow-up activities, including agendas, meeting materials, summaries, and accountability tracking.
• Work closely with the Close to Home Communications Director and ACAC to support the HRAP communications strategy by providing project updates, coordinating timelines, documenting successes and lessons learned, and supporting audience-specific communications.
• Work with SAHT, Close to Home, and Communications to maintain the HRAP funding/fundraising tracker and connect funding progress to implementation sequencing.
• Support onboarding of newly funded HRAP projects and service providers into implementation, governance, Homelink, communications, HMIS and performance reporting expectations.
• Develop HRAP implementation processes, flow charts, and coordination structures.
• As appropriate, participate in RFP, procurement, or service-provider selection processes associated with HRAP implementation.
• Support identification of HRAP implementation and outcome measures and integration of those measures into existing Alliance monitoring and reporting structures.
• Support integration of HRAP activities, newly funded projects, performance measures, lessons learned, and recommended system improvements into existing Alliance governance, Homeless Response System operations, and performance monitoring structures.
• Prepare implementation updates, executive briefing materials, facilitation summaries, and other documentation necessary to maintain momentum and accountability.

Pricing
Anticipated hourly rate range: $100–$150 per hour, commensurate with experience. Provide your 
hourly rate, estimated availability of 20–25 hours per week, and any anticipated reimbursable expenses.

Work Location & Schedule
This is a local opportunity based in San Antonio / Bexar County. The role is hybrid. Work-from-home is 
allowable, but regular in-person attendance at meetings, working sessions, and community 
engagements will be required.

Evaluation Criteria

Responses will be evaluated based on:

• Relevant experience with street-to-home, housing-focused, or coordinated homelessness response initiatives.
• Project and implementation management experience, particularly leading multi-partner or systems-level initiatives from planning through execution.
• Demonstrated facilitation and collaborative leadership, as reflected in work samples, references, and the candidate's described approach to building consensus among diverse partners.
• Strategic thinking and problem-solving, including the ability to translate operational learning into system-level recommendations.
• Communication and organizational skills, including the clarity and quality of the Letter of Interest and submitted work samples.
• Experience with governance, performance monitoring, and systems implementation in housing, homelessness, or other complex human service systems.
• Overall fit for Project DC, including approach to the first 90 days and alignment with the initiative's scale and community context.
