ABOUT THE POSITION

North Shore CDC is seeking a compassionate, resourceful, and highly motivated Resident Services Case Manager to provide direct support, outreach, case management, resource navigation, and community programming to residents and community members experiencing housing instability. This position will work with a diverse range of residents, including young adults, families, seniors, and individuals who are experiencing or at risk of homelessness or housing instability. The Case Manager will help individuals identify their goals, navigate complex systems, access community resources, and develop plans that promote housing stability, wellbeing, and long-term success.

The ideal candidate is someone who believes in meeting people where they are, takes a strengths-based and person-centered approach, and is committed to empowering individuals to identify and achieve their own goals. This position requires someone who can build trusting relationships, manage multiple cases and priorities, respond effectively to crises, and work collaboratively with property management, community partners, and service providers. The Coordinator will serve as an important connector between residents, NSCDC staff, property management, service providers, community organizations, and other resources. The position will also support community-building efforts by coordinating and facilitating workshops, activities, and other programs that strengthen resident connection, wellbeing, and access to opportunity.

KEY RESPONSIBILITIES

Case Management & Resident Support

• Develop and maintain supportive, trusting relationships with residents and community members.

• Lead extensive outreach efforts and connect with those, specifically youth, experiencing homelessness or at risk of becoming homeless 

• Conduct intake and needs assessments to understand individual strengths, needs, goals, and barriers.

• Develop individualized service and/or goal plans in collaboration with participants.

• Provide ongoing case management, coaching, follow-up, and stabilization services.

• Help residents access and navigate housing, employment, education, healthcare, public benefits, transportation, food access, and other community resources.

• Provide referrals and coordinate services with community-based organizations, healthcare providers, employment programs, educational institutions, and other partners.

• Provide crisis intervention and help individuals develop emergency plans and connections to appropriate shelter or other resources when necessary.

• Advocate with and on behalf of residents while promoting self-determination and independence.

Outreach & Community Engagement

• Conduct outreach to individuals and households who may benefit from resident services or housing stability supports.

• Build relationships with residents in NSCDC housing and with community members seeking assistance.

• Participate in community outreach activities, events, workshops, and other engagement opportunities.

• Identify emerging resident needs and opportunities for new services, resources, and partnerships.

• Build relationships with community organizations, state and federal agencies, businesses, and other partners to expand opportunities and resources available to residents.

Programming & Community Building

• Coordinate and facilitate on-site programs, workshops, and activities that promote wellness, social connection, education, employment, and lifelong learning.

• Partner with local service providers and community organizations to bring educational, health, workforce, and other programming to residents.

• Encourage resident engagement and support resident-led activities and initiatives.

• Assist with or coordinate interns, fellows, volunteers, and other individuals supporting resident services activities.

Collaboration & Communication

• Maintain regular communication with property management staff regarding resident needs and concerns while maintaining appropriate confidentiality and professional boundaries.

• Collaborate with NSCDC staff and external partners to coordinate services and respond to resident needs.

• Participate in case conferences, team meetings, supervision, trainings, and professional development activities.

• Communicate effectively with residents, colleagues, property management, service providers, and community partners.

• Use a culturally responsive, trauma-informed, strengths-based, and person-centered approach in all interactions.

Documentation, Data & Reporting

• Maintain accurate, timely, and confidential records of resident interactions, assessments, referrals, services, and outcomes.

• Complete progress notes and case documentation in accordance with agency requirements.

• Collect intake, eligibility, and other required program documentation.

• Track services, demographics, outcomes, and participant progress.

• Participate in reviewing participant trends and identifying opportunities to improve service delivery.

• Prepare reports related to program activities, outcomes, and goals.

• Support grant compliance and reporting requirements through accurate documentation and data collection.

• Participate in regular case review and consultation to ensure effective follow-up and coordination of services.

QUALIFICATIONS

• 5+ years of experience in social services, senior services, resident services, or a related field preferred.

• Working knowledge of affordable housing programs such as Low-Income Housing Tax Credit (LIHTC), RAFT or HUD related programs.

• Familiarity with tenant income recertification process and administrative functions for individuals within the affordable housing community.

• Ability to work independently and manage time effectively in a part-time role.

• Strong interpersonal, relationship-building, organizational, and written communication skills.

• Compassionate, patient, strengths-based, person-centered and culturally responsive approach to working with diverse senior populations.

• Comfortable facilitating groups, workshops, and community activities.

• Bilingual (Spanish or other languages common in the community) is a plus.

• Occasional evening events may be required; flexible scheduling is available.

• Daily access to an automobile and a valid Massachusetts driver’s license may be required for community-based outreach and services.
