Job Details: Job Location: Galesville - Galesville, WI, Position Type: Full Time, Salary Range: Undisclosed, Join our team as a Rehab Director – Occupational Therapist or Occupational Therapist Assistant 

Greenfield Rehab is seeking a full-time experienced and dedicated Occupational Therapist (OT) or Occupational Therapist Assistant (OTA) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Galesville, WI.  

The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Occupational Therapist or Occupational Assistant, providing direct patient care as needed.

Strong clinical reasoning and patient identification skills.

Occupational Therapist Qualifications

Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Occupational Therapy.
License as required by the State of Wisconsin to practice as an Occupational Therapist.

Occupational Therapist Assistant Qualifications 

Graduate of an accredited school with an associate degree as a an Occupational Therapy Assistant.
Certification or license as required by the State of Wisconsin to practice as Occupational Therapy Assistant.
