Registered Nurse-Home Health

Central Coast Home Health and Hospice is a locally owned, rapidly growing home health organization proudly serving patients throughout San Luis Obispo County and Santa Barbara County. We are seeking compassionate and motivated Registered Nurses to join our team in multiple territories across San Luis Obispo County in a variety of settings:  Home Health or Palliative Care or Hospice.

Home Health RN: You will provide high-quality, patient-centered care in the comfort of patients’ homes while collaborating with an interdisciplinary care team. This role offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families while managing your own schedule and caseload.

Palliative Care RN:  Cares for patients with life-limiting diseases who are still seeking curative treatments and interventions.  The focus is on symptom management, advance directives and other end of life decisions, and education regarding disease processes.   The Palliative Care nurse works collaboratively with a multidisciplinary team including social workers, spiritual counselors and physical/speech/occupational therapists. By seeing your patients in the home, you are able to spend the time necessary developing relationships, answering questions and assessing the needs of the patient and family on a psychosocial, physical and spiritual level. 

Hospice RN: Plans, organizes and directs hospice care while building community resources to plan and direct services to meet the needs of the patient and their families within their homes and communities. Collaborating with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients.

Position Details

• Full-time or part-time opportunities available (minimum 3 days/week)
• Flexible scheduling with choice of 4- or 5-day workweek
• Standard schedule: Monday–Friday, 8:00 AM–5:00 PM
• Rotating weekend coverage required

Responsibilities

• Deliver skilled nursing care in patients’ homes
• Assess patient needs and develop individualized care plans
• Coordinate care with physicians and interdisciplinary team members
• Complete timely and accurate clinical documentation
• Educate patients and caregivers regarding treatment plans and health management
• Promote continuity of care and positive patient outcomes

Qualifications

• Current California Registered Nurse (RN) license
• Minimum one year of professional nursing experience
• Current CPR certification (Healthcare Provider)
• Valid driver’s license with clean driving record
• Reliable transportation and current auto insurance
• Ability to work independently and manage a flexible caseload
• Home Health/Palliative Care/Hospice experience preferred but not required
