Registered Nurse - Transfer Center - PRN

The Registered Nurse answers calls to collect clinical information relative to a transfer request and facilitates the transfer by matching the patient's clinical need to the facility with service and bed capacity capability. The major responsibilities of the role are: provides status reviews for all patients from any portal of entry (planned or emergent) prior to bed assignment; performs initial review based on the status specified by the physician's order and will follow through with analyzing clinical information and making recommendations regarding appropriateness of care; assures the financial integrity of the organization by decreasing waste by assuring proper admission status is communicated to registration and care managers, that information regarding current admission is adequate to meet screening criteria or to communicate with physician and / or physician advisor when criteria for admission are not met. Assesses the patient to ensure the right care is delivered at the right time by identifying delays and removing barriers.

• Assesses patient needs and coordinate appropriate level of care and bed placement to ensure safe, timely admissions and transfers.
• Facilitates inbound and outbound transfers, ensuring efficient communication and seamless coordination between referring and receiving providers and facilities.
• Serves as a central point of contact for physicians, facilities, and internal teams to support direct admits, ED transfers, and specialty activations.
• Ensures adherence to clinical protocols, regulatory standards, and patient safety practices throughout the transfer and admission process.
• Maintains accurate documentation, track key metrics, and support process improvements to enhance patient flow, timeliness, and service performance.

Qualifications

Qualifications

Minimum Education
 

• Technical Diploma/Certificate Nursing Required or
• Associates Degree Nursing Required
• Bachelor's Degree Nursing Preferred

Minimum Work Experience
 

• 5 years Nursing experience in an acute care hospital setting. Required

Licenses and Certifications
 

• Registered Nurse Licensed State of Florida or eligible compact state Upon Hire Required
• BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required

• Demonstrated proficiency in acute care nursing, knowledge, and skills.
• Basic Computer Skills
