Job Description:

The Registered Nurse (RN) collaborates with interdisciplinary care team members to deliver high-quality, patient-centered care. This role is responsible for assessing patient needs, developing and implementing care plans, coordinating care throughout the patient's hospital stay, providing patient and family education, and supporting positive clinical outcomes, patient satisfaction, and safe discharge planning.

Essential Responsibilities

Patient Care & Clinical Practice

• Provide comprehensive nursing care utilizing the nursing process, evidence-based practice, and established clinical standards.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Monitor patient progress and document care, interventions, and outcomes accurately and timely.
• Administer medications and treatments in accordance with hospital policies, procedures, and regulatory standards.
• Recognize and respond to changes in patient condition, escalating concerns as appropriate.
• Maintain patient safety and promote quality outcomes through adherence to established protocols and best practices.

Care Team Collaboration

• Partner with physicians, nurse coordinators, case managers, and other healthcare professionals to ensure coordinated, efficient patient care.
• Participate actively in interdisciplinary rounds, team meetings, and quality improvement initiatives.
• Communicate patient status, care needs, and clinical information effectively with appropriate team members.
• Serve as a clinical resource for colleagues and assist in resolving complex patient care issues.
• Foster a professional, respectful, and collaborative working environment.

Patient & Family Education

• Provide individualized education to patients, families, and caregivers regarding diagnoses, treatment plans, medications, self-care, and discharge instructions.
• Assess patient and family learning needs and adapt education accordingly.
• Support the development and delivery of patient education materials and resources.
• Ensure patients and caregivers understand available community resources and post-discharge care requirements.

Care Coordination & Discharge Planning

• Collaborate with the care team to manage patient care based on anticipated length of stay and clinical goals.
• Coordinate procedures, diagnostic testing, consultations, and treatments to support timely care progression.
• Participate in discharge planning beginning at admission to ensure safe and effective transitions of care.
• Work closely with case management and other departments to facilitate appropriate discharge plans and utilization of resources.
• Ensure discharge instructions and follow-up plans are provided and documented prior to patient discharge.

Quality Improvement & Professional Development

• Participate in quality assurance, patient satisfaction, and performance improvement initiatives.
• Monitor patient outcomes and contribute to the development of strategies that enhance quality and safety.
• Support staff development and education activities to promote clinical competency and excellence in patient care.
• Maintain current clinical knowledge through ongoing professional development and continuing education.

Required Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) licensure in the Commonwealth of Massachusetts.
• Strong knowledge of nursing principles, patient assessment, care planning, and clinical practice standards.
• Excellent communication, interpersonal, and collaboration skills.
• Ability to work effectively with patients, families, and interdisciplinary healthcare teams.
• Demonstrated critical thinking, problem-solving, and organizational skills.
• Commitment to continuous learning and professional development.

Preferred Qualifications

• Bachelor's degree in Nursing (BSN) preferred.
• Previous acute care or specialty nursing experience preferred.
• Experience working in a collaborative, team-based healthcare environment.

Pay Range:

$37.00 - $95.37

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
