Glendive Medical Center is seeking a compassionate and skilled Registered Nurse (RN) to join our Hospice and Visiting Nurse Service team on a PRN basis. This role provides nursing care to patients receiving hospice, home health, and chronic care management services throughout our community.

As a PRN RN, you will support patients and families by delivering high-quality, patient-centered care in their homes and other community settings while collaborating with an interdisciplinary team dedicated to enhancing quality of life.

What You'll Do

• Perform comprehensive patient assessments and document findings accurately.
• Develop, implement, and evaluate individualized nursing care plans.
• Coordinate care with physicians, patients, families, and interdisciplinary team members.
• Monitor patient conditions and communicate significant changes appropriately.
• Participate in interdisciplinary team meetings and care conferences.
• Provide guidance and support to LPNs and other healthcare staff.
• Assist with Medicare Annual Wellness Visits, Dementia Team Assessments, and COVID follow-up visits and calls as needed.
• Maintain compliance with department policies, procedures, and nursing standards.
• Participate in continuing education and professional development opportunities.
• Perform other duties as assigned.

On-Call Responsibilities

PRN nurses may have opportunities to participate in the department's on-call rotation based on staffing needs. On-call responsibilities include responding to patient needs and providing nursing support during assigned on-call periods

Qualifications

Required

• Current Registered Nurse (RN) license in the State of Montana.
• Minimum six months of recent professional nursing experience.
• Current BLS Certification.
• Valid driver's license and ability to travel throughout the service area.

• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Effective organizational and time-management abilities.
• Ability to work independently and as part of a multidisciplinary team.
• Leadership skills with the ability to supervise and delegate appropriately.
• Proficiency with Microsoft Office and electronic health documentation systems.

Physical Requirements

• Frequent standing, walking, bending, and reaching.
• Ability to lift up to 35 pounds.
• Ability to travel throughout the service area and safely drive in varying weather conditions.
• Ability to work flexible hours and participate in on-call coverage as needed.
