Job DetailsJob Location: Good Shepherd Lutheran Home - Sauk Rapids, MN 56379Position Type: Full-timeSalary Range: $35.65 - $43.87 HourlyTravel Percentage: OccasionalJob Shift: Day ShiftJob Category: NurseThe RN Float is responsible for assisting with the provision of care and services to clients receiving Assisted Living services, Assisted Living with memory care services, or services through the Good Shepherd Comprehensive Home Care license. Provides nursing leadership, direction, and supervisory oversight to licensed/unlicensed staff. Will be engaged in client pre-screening/admission process, developing a person-centered plan of care, and ongoing client assessments. Routinely engaged in ensuring plan of care for each client is up to date, processed, and communicated timely. Coordinates services ensuring coordination of care for each client and meets continued stay criteria. Provide direction to staff on engaged communication to clients with memory loss. Actively engaged in QAPI initiatives, develops best practices, assists with staff education, and ongoing policy development.

Assists area RNs in developing and updating a person-centered plan of care for each client after assessing/reassessing clients’ medical, nursing, and psycho-social needs by:

Conducts client pre-screenings and admissions based on current continue stay criteria.
Develops a person-centered plan of care based on assessment and client/responsible party wants/needs.
Coordinates/modifies plan of care/orders as needed with physician, as defined by policy.
Directs, supervises, and teaches staff on services noted in the plan of care.
Provides nursing services according to an up to date plan of care.
Consults with physician for approval/modification of plan/orders, per policy as needed.
Assists in coordinating additional community referral services as needed.
Generates and monitors an accurate clinical record and physician updates, as defined by policy.
Engages families/agencies as changes in condition occur that may impact continued stay criteria.

In conjunction with area RNs, monitors and directs all aspects of medication administration and/or self-administration.

Reviews and monitors all medication of clients and directs the proper administration.
Ensures medications and order changes are properly processed, documented, properly signed by medical professional, ordered, delivered, etc as per physician and plan of care.
Ensure medical/nursing equipment and materials working properly for needed interventions/treatment.
Administers medications/injections, as may be needed/ordered, as defined by policy.
Monitors medications and medication regime, as defined by policy, and contacts prescribing professional as needed.
Monitors scheduled/all medication and follow drug diversion and awareness process.

Demonstrates a working knowledge of current nursing practices when performing observation, nursing assessments, clinical updates, post-fall follow-ups, and all mandated processes per regulation.
Demonstrates the ability to routinely assess and assimilate client needs and follow-up with appropriate nursing interventions, timely documentation, and staff training.
Coordinates and directs the provision of delegated nursing services to nursing/unlicensed staff in compliance with current licensure.

Directs and provides nursing supervision of licensed/unlicensed staff involved in the provision of Assisted Living Home Care services.
Will assist/engage in staff supervisory visits, evaluations, coaching, and disciplinary matters as needed.
Maintains ongoing awareness of state mandated and client services training needs.
Will be engaged in on-going training processes based on client needs, policy, and regulations.
Assists new nursing personnel to learn policies, procedures, and best practices.

Fosters respectful and engaged communication techniques with clients with memory loss and leads/directs staff in proper interventions with all clients.
Establish/maintain strong communications with families/caregivers and all agencies involved in client care including but not limited to: case managers, home care, hospice, pharmacy, care centers other Assisted Living’s, and any other health care professional to provide a continuity of client care.
Monitors and acts upon routine nursing data, notes, messages, and client/family calls to ensure client care plan reflects ever-changing needs.

Checks vital signs.
Monitors skin integrity.
Monitors/assesses fall and other risk hazards and mobility factors.

 Assist with direct care client services to ensure client plan of care is met.

Assists/teach client appropriate self-care techniques.
Visits with and rounds on clients routinely.
Assist clients with ADL’s, personal, and special cares.
Assist with bowel and bladder needs.
Assist with transfers and escorts.
Encourage/assist with activity, exercise, and psycho-social options.

Coordinates/arranges for needed services.

Monitors client care and contacts additional supports/services to maximize client care including but not limited to: Home Care, PT/OT services, Hospice, etc.
Ensure there is a process to arrange for doctor’s visits, clinic appointments, etc.
Directs/arranges/follows-up on transfers to hospital and as defined by policy.
Responds/assists with emergencies.

Practices and helps to maintain a safe environment for the client and staff.

Practices/promotes proper infection control practices.
Maintains up to date knowledge of all emergency procedures.
Maintains up to date knowledge of the Good Shepherd Abuse Prevention, Corporate Compliance, and other mandated processes.
Directs staff in correct procedure and notifies assigned personnel per policy/best practice.

Participates in RN On-Call process as assigned.

 Responds to RN calls in a timely manner.
 Practices proper nursing judgement while assessing call information.
 Communicates expectations to unlicensed staff about the interventions they need to follow.
 Follows-up and documents timely on all calls.

Fosters an environment that engages team building, safe discussion, questions, high levels of communication, etc. to ensure exceptional care and promote employee retention.
Communicates effectively and respectfully with all members of the team.
Actively participates in the agency Quality Assurance program. Works with supervisor to establish goals that will impact client care, promote Quality Assurance, foster improved staff education, etc. that help to fulfill the mission and values of Good Shepherd.
Engages in staff/team meetings and follow-up on assigned duties.
Completes mandated educational requirements and maintains licensing requirements for position.
Assumes responsibility for professional growth and development by actively participating in continuing education, in-service programs, orientations, and personnel evaluations.

QualificationsRN Float Qualifications:

Have graduated from an accredited school of professional nursing.
Have current licensure as a Registered Nurse in the State of Minnesota.
Have a current Basic Life Support (CPR and AED) certification.
Have at least one (1) year experience in an acute hospital or equivalent experience.
Exhibit professional behavior and attitude.
Demonstrates the ability to make appropriate professional judgments.
Possess good interpersonal communication skills.
Valid driver’s license required.
Must demonstrate leadership skills necessary to perform the responsibilities of the position.
