Registered Nurse (RN) Discharge Lounge
Monday-Friday (12 hours per week. Can be split between 8- and 4-hour shifts)

ChristianaCare Hospital in Newark, DE is currently seeking an RN to work in our Acute Care Discharge Lounge. In this highly visible role, the RN will be responsible for assisting the discharge of patients and partnering with our providers to ensure a safe handoff to their caregivers. This is an opportunity to be involved in the frontline care continuum to ensure a safe handoff to the community.

PRIMARY FUNCTION:

A Registered Nurse (RN) is a health care provider who implements skilled nursing care to adult and geriatric patients, while observing for patient response to prescribed treatments and/or nursing interventions—within the practice scope as delineated in the Delaware Nurse Practice Act.

• Conducts ongoing patient observations to gather data relevant to the patient needs recording significant conditions and response to treatment, notifying assigned RN of any changes in condition.
• Performs assigned nursing procedures for comfort and well-being of patients demonstrating a concern for patient safety.
• Participates in patient/family education and discharge planning, documenting appropriately.
• Updates the teaching plan as appropriate.
• Establishes priorities appropriately, based on patient needs.
• Demonstrates skills and knowledge necessary to provide care to adult and geriatric patients including knowledge of growth and development, the ability to obtain information and to provide the care needed.
• Communicates effectively to patients, families, and the health care team, utilizing both written and verbal communication skills.
• Responds to patient crisis situations as outlined in unit specific standards. 
• Performs assigned work safely, adhering to established CCHS and departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients, and visitors.
• Completes mandatory in-service education.
• Performs other related duties as required.

SPECIAL REQUIREMENTS:

Current RN licensure in the state of Delaware. 

BLS certification.

PHYSICAL DEMANDS:

Ability to stand, walk, sit, bend, lift, push, pull, carry, teach, twist, handle, stoop, climb as per unit based Physical Demands Checklist.

WORKING CONDITIONS:

Frequent exposure to biological elements with occasional exposure to chemical and environmental elements as per unit-based Physical Demands Checklist.

Hourly Pay Range: $42.11 - $67.37

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
